Description |
The Young People's Throughcare Team provides guidance and support, both practical and emotional, by actively involving young people who are being or were looked after away from home on a residential basis. The Team: are involved in the planning and preparation required to enable the young person to live independently offers advice and information for young people helps young people move on and establish themselves in the community keeps in touch with young people as long as is required is involved in establishing links and joint working with other agencies |
